According to a report conducted by Milliman, consulting group, hospital errors in the United States result in $19.5 billion in costs and 2,500 excess deaths.  Researchers reporting in 2008 that were 1.5 million injuries to medical errors.

This eye-opening report defined a medical error as "avoidable negative result of medical care, which was by due to lack of care by improper medical management, a failure of the Commission) rather than a progression of the disease (an error sins of omission)."
