According to a report conducted by Milliman, consulting group, Hospital errors in the United States resulting in the amount of $ 19.5 billion cost and 2,500 excess deaths.  Researchers, who in 2008 reported medical errors concerned 1.5 million injuries.

This eye opening report defined a medical error as "avoidable negative result of medical care, which was by due to lack of care through improper medical management (an error of the Commission) rather than a progression of the disease (an error sins of omission)."
The medical errors are quite extensive, but the researchers could "top 10" list of 69 per cent of all medical errors to compile a .  This list contains:
